How To Get Your South Dakota Medical Card

We understand that these regulations may be changing but as of the publishing date (January 27th, 2023), the current guidelines remain in effect.

Step 1: Find a Provider and Book An Appointment

Step 2: Paperwork

You should receive some type of paperwork in your email asking you to fill out a patient questionnaire after booking your appointment. Complete the electronic paperwork.

Step 3: Medical Records

You will need to provide medical records supporting your qualifying condition in order to get certified for your medical card. Most patients obtain their records through one of the following online wellness portals or apps:

– MyChart

– Follow My Health

– My Health Vet (VA)

The most common forms of medical records provided by patients include:

– Office Visit Summaries

– Treatment Notes

– Letters From Your Current Physician

– Physician Referrals

Step 4: Get Evaluated and Certified by a State Certified Practitioner

Meet with a provider to review your medical history and qualifying condition. If your physician determines that you’ll benefit from this treatment, they will issue a Physician Certification. Following your appointment, the practitioner will submit that Physician Certification Form to the State of South Dakota through an online portal.

Step 5: State Sign-Up

Step 6: Get Your Card

The DOH has 15 days to approve or deny your application if approved your Medical Card will be mailed to you. After receiving your card, you can legally purchase from any South Dakota dispensary. https://thecoughyhouse.com/dispensaries/

Qualifying Conditions:
  • Cachexia or wasting syndrome – Severe, debilitating pain
  • Severe nausea
  • Seizures
  • Severe and persistent muscle spasms, including those characteristic of Multiple Sclerosis
 
South Dakota Medical Card Requirements:
  • Be 18 years of age or older. Patients under 18 years of age must have parental consent.
  • Have a current diagnosis of a qualifying condition.
  • Be a South Dakota resident with proof of residency (Valid South Dakota driver’s license or state ID card
